SinfoníaRx has partnered with the American Pharmacists Association (APhA) to provide a complementary lifestyle change program to help prevent type 2 diabetes and promote healthy living. This program is funded by the Centers for Disease Control and Prevention (CDC) through an award from the APhA Foundation.

Take control of your health to prevent diabetes

The National Diabetes Prevention Program offers participants on-demand access to telehealth sessions facilitated by lifestyle coaches.

What can be expected of the program?

Who is eligible to participate in the program?

a. A blood test result in the prediabetes range within the past year via ONE of the following:

1. Hemoglobin A1C: 5.7%–6.4%
2. Fasting glucose: 100–125 mg/dL (or 110 to 125 if Medicare recipient)
3. Two-hour plasma glucose (after a 75 gm glucose load): 140–199 mg/dL
4. Previously clinical diagnosis of gestational diabetes during previous pregnancy

b. CDC/American Diabetes Association Prediabetes Screening Test score of 5 or higher
