SinfoníaRx is partnering with the American Pharmacists Association (APhA) Foundation to provide a complimentary lifestyle change program to help prevent type 2 diabetes and promote healthy living. This program is funded by the Centers for Disease Control & Prevention (CDC) through an award to the APhA Foundation.

Take control of your health to prevent diabetes

The National Diabetes Prevention Program presents participants with on-demand access to topical telehealth sessions facilitated by trained Lifestyle Coaches.

What can be expected of the program?

  • Convenience and confidentiality
  • Helpful tips for healthy living
  • Reduction of risks associated with type 2 diabetes
  • A personal lifestyle coach

Who is eligible to participate in the program?

  • Adult (18 years or older)
  • Not pregnant at time of enrollment​
  • BMI ≥ 25 (≥ 23 if Asian)
  • Have no previous diagnoses of type 1 or type 2 diabetes
  • Qualify for the program based on either:

a. A blood test result in the prediabetes range within the past year via ONE of the following:

1. Hemoglobin A1C: 5.7%–6.4%
2. Fasting glucose: 100–125 mg/dL (or 110 to 125 if Medicare recipient)
3. Two-hour plasma glucose (after a 75 gm glucose load): 140–199 mg/dL
4. Previously clinical diagnosis of gestational diabetes during previous pregnancy

b. CDC/American Diabetes Association Prediabetes Screening Test score of 5 or higher
